This is a reference to the original Primalist's Seal, which dropped from Bloodlord Mandokir in the original Zul'Gurub.

If a maxed out healer or shadow priest, but you don't do current expansion normal/heroic raids, these will likely either replace your Leven's Circle of Hope, or save you from buying them. Alternately, if you got up to Exalted with the Klaxxi and did the final quest, it will replace your Seal of the Prime.
